How to apply for a duplicate PAN Card in case of Lost and Damaged

• Second Step: Take a print out of the filled form and paste your photographs.

• Third Step: Now, send the completely filled & signed application long with all required documents to the PAN Service Office. 

Following is the list of documents which is required to apply for a duplicate PAN card

• You will require two stamp size photographs.

• A proof of your previous PAN Card that are

• • Copy of your previous PAN Card

  • Copy of PAN Allotment Letter

  • And most importantly copy of FIR which states that you have lost your PAN card.

• Identity Proof (any one from the following list having full name)

• • Driving License

  • Voter Identity Card

  • Passport

  • School Leaving Certificate

  • Matriculation Certificate

  • Degree of Recognized Educational Institution

  • Passbook along with transactions for last 3 months

  • Bank Account Statement transactions for last 3 months

  • Depository Account Statement

  • Water Bill

  • Property Tax Assessment Order

  • Certificate of Identity signed by a Gazetted Officer / MLA / MP/ Municipal Councillor.

• Address Proof (any document from the above mentioned list)

• Date of Birth Proof (from the following documents)

Note: You are not required to submit the damage PAN card but copy of FIR is mandatory to submit while applying for a duplicate PAN card.
